Immunization
At iKids we understand that your personal health is very private, and we respect that.
iKids health & safety guidelines, requires an Immunization Form 3231 or Affidavit of Religious Objection Form 2208 (revision dated 6/2019) issued by your pediatrician to be kept on file for all registered students. The form can be uploaded, or a copy can be dropped off to our offices before your child will be able to attend any iKids activities.
